Then, complete the form by checking your choices and sign it. Then, you need to designate the names and contacts of the two people you have chosen to speak for your, your “agents”. You need to tell them what your wishes are and provide them with a copy of the document you have signed and dated. NOW, you have a legally recognized Advanced Directive.
